I spent a great deal of time trying to figure out why I was seeing the same issue as above, and then realized the enemy NPC was standing in an unlit area and being considered hidden. I wish there was a way for the DM to see more info on why adv, dis etc are being pulled on the info card.

I've managed to set up the concentrator on Midi QOL pretty well, except for one thing. The concentration check doesn't happen if the target only takes temporary hp damage. This only works for spells that target enemies though, or it will target the caster too. Second, you can clear the target portion and just use the range, then shift click manually target each creature. This is a little slower, but gives you more control over the targeting.
